Overview


The ALSA is responsible for the facilitation and coordination of all requests relating to patient referral, physician consultation and patient transfer. The primary job responsibilities of the Transfer Coordinator are to receive and process telephone calls requesting assistance with acute patient transfers and/or admissions of patients from medical facilities. During downtime the patient transfer center paramedic assists registered nurse in the assessment of the health status of Emergency Department patients. Transport monitored and non-monitored patients to inpatient units and diagnostic areas. Checks patient's vital signs. Prepares, hangs, monitors and discontinues I.V. fluids.

Responsibilities

*Complies with PENN HIGHLANDS DUBOIS policies and procedures, accreditation agency requirements, and federal, state or local law and regulations

 
Demonstrates competency in Patient Transfer Paramedic role as evidenced by but not limited to:

§  Receives physician requests from transferring facilities and coordinates patient transfer to receiving facility

§  Professionally and accurately represents Penn Highlands services

§  Develops and maintains referral relationships with outlying facilities through consistent customer service and ease of use

§  Facilitates all communications between referring and receiving physicians and other hospital staff

§  Documents all required information pertaining to the patient transfer process

§  Accurately enters data into the database as it occurs for creation of real time data

§  CONTRIBUTES INFORMATION TO A REGISTERED NURSE TO THE ASSESSMENT OF THE HEALTH STATUS OF EMERGENCY DEPARTMENT PATIENTS. 

§  TRANSPORT MONITORED AND NON-MONITORED PATIENTS TO INPATIENT UNITS AND DIAGNOSTIC AREAS. 

§  CHECKS PATIENT'S VITAL SIGNS.  PREPARES, HANGS, MONITORS AND DISCONTINUES I.V. FLUIDS. 

 

 
Participates in the development of new staff as evidenced by but not limited to:

Actively participates in the orientation including cultural and social integration of new staff
 

 

 
Demonstrates personal accountability as evidenced by but not limited to:

Achieves educational credits to meet hospital requirements
Shares information obtained at education conferences to staff members
Participates in implementing departmental initiatives
Conducts self in professional manner in dress, actions, and interpersonal communication
Ensures environment of safety for patients, significant others, and coworkers
Informs Director/Supervisor/Clinical Manager of patient care situations as appropriate
Demonstrates effective problem solving skills for departmental issues
 
General duties as directed

 

Qualifications

  • HIGH SCHOOL DIPLOMA OR EQUIVALENT REQUIRED; COMPLETION OF POST HIGH SCHOOL SECRETARIAL PROGRAM PREFERRED
  • PREVIOUS EXPERIENCE IN OFFICE/SECRETARIAL SETTING PREFERRED
  • SUCCESSFUL COMPLETION OF A PARAMEDIC TRAINING COURSE REQUIRED
  • MUST HAVE AND MAINTAIN A PA PARAMEDIC CERTIFICATION
  • ACLS AND PALS CERTIFICATION REQUIRED
  • TYPING AND COMPUTER SKILLS
  • ABILITY TO PRIORITIZE AND MULTI-TASK DUTIES
  • ABILITY TO EFFECTIVELY COMMUNICATE WITH COWORKERS AND PHYSICIANS
  • FAMILIARITY WITH MEDICAL TERMINOLOGY PREFERRED
